A Hospice agency can refer a patient to the Hyder Hospice Unit. And, physicians are welcome to follow their patients after a referral. To make a referral,

We can answer your questions and make arrangements for a hospice nurse specialist to visit and assess the patient’s needs.
Our hospice services are available after a hospitalization, when the care needed exceeds other discharge planning options. You and your loved ones might consider a stay at the Hyder Hospice Unit, in Dover, N.H., if: |

An assessment of each patient determines if inpatient hospice care is the best option for your care. And, the Hospice care team, along with doctors and nurses currently involved in the patient’s care, will determine if the Hyder Hospice Unit is the right place for you and your family. Patients may be admitted for a short stay in order to receive pain and symptom management and intensive palliative care before returning to their home. |
Families are welcome to gather together for meals in our dining room. Food can be stored in our kitchen or ordered from local restaurants. |
Our community room offers space for various activities, and for private meetings with family and others involved in your loved one’s care. |

To make your stay here at the Hyder Hospice Unit as comfortable as possible, we encourage patients to bring their personal items, such as photographs, pillows, CD players, quilts, and clothing along with them. And, with staff coordination, we welcome pets for supervised visits. |
